The Air Merpati flight I caught to Lombok in June 2002, was full, as was the last flight back that same day. Not sure if it's as busy these days.
I tried to book over the phone in Bali with Air Merpati, but kept getting put thru' to the wrong extn, or was cut off. After 6 attempts, I got the Business Office in our hotel to organise it for me. Cost me 5000Rp ($1 approx), and ticket was brought to the hotel, and I paid for it then. So guess, it's up to you how you decide to organise it. I have great driver recommendation on Lombok if you want to use same.
