I recommend staying at Ketut Bungalows for all that you listed and more. Ketut, the owner, is friendly and nothing is too much of a hassle for him or his staff.
I paid 60,000 rupiah/night for a new room, the older rooms are 40,000/night. The rooms are large and the bed is massive-bigger than a king size! They have a fan and are very clean. On the beach and close to the good surf and restaurants.
